We provide consistent, high-quality medical care for adults and families — focused on prevention, long-term health, and managing everyday medical needs. Our approach to care is simple: listen carefully, treat with respect, and make care accessible to everyone.
If you do not have insurance or cannot afford to pay, we will still see you and help you.
What Is Primary Care?
Primary care is your first stop for most health concerns. It’s where ongoing relationships are built and where health issues are addressed early — before they become more serious.
Our primary care team gets to know you, your health history, and your goals, so care feels personal and consistent over time.
Primary Care Services
We provide a wide range of medical services, including:
- Preventive care and wellness visits
- Annual physicals and routine checkups
- Management of chronic conditions such as diabetes, high blood pressure, and asthma
- Care for acute illnesses and minor injuries
- Lab work and diagnostic services
- Medication management
- Referrals and coordination with specialty care
When needed, we coordinate specialty services such as cardiology, pulmonology, and endocrinology through partnerships, including Mayo Clinic.
Whole-Person, Team-Based Care
Your health is more than a single visit. Our primary care providers work closely with behavioral health, dental, and other services to make sure care is connected and practical.
This team-based approach helps address physical health, mental health, and social factors together — all in one place when possible.

We have been designated as the first Foster Care Center of Excellence by Sunshine Health in Northeast Florida. The Foster Care Center of Excellence (FCCOE)designation is awarded to medical centers that provide expert, trauma-informed care for families involved in the child welfare system. To receive this accreditation ,medical centers must meet 20 rigorous criteria.
Angela’s Story: From Crisis to Stability

When Angela first came to Duval Family Health Center nearly two years ago, she was 44 years old and facing one of the most difficult periods of her life.
She had recently been released from incarceration and was living temporarily with friends, unsure how long that stability would last. She had no job, was uncertain about her insurance status, and had not had consistent primary care in years. Much of her recent medical care had taken place in emergency rooms and hospitals, often during moments of crisis.
Angela’s health challenges were serious and complex. She was living with heart failure, high blood pressure, COPD, sleep apnea, and a history of cocaine use. Her heart function was significantly reduced, placing her at risk for dangerous heart rhythms, and she depended on a wearable cardiac defibrillator (LifeVest). She experienced frequent shortness of breath, chest pressure, leg pain, and repeated hospital visits. Managing her health felt overwhelming, and trust in the healthcare system was fragile.
From the very beginning, our team focused on more than just medical conditions.
At Duval Family Health Center, Angela was met with consistency, compassion, and respect. Providers worked closely with her to rebuild continuity of care, coordinate cardiology follow-up, ensure access to medications, and support healthier daily routines. Just as important, she received encouragement and support around substance use recovery — without judgment.
Over time, Angela’s commitment and the support around her made a real difference. She achieved and maintained sobriety, remaining cocaine-free. With consistent medical care and follow-up, her heart function steadily improved. As her condition stabilized, testing showed significant recovery — so much so that she no longer needed the LifeVest that once symbolized how vulnerable her health had been.
Today, Angela’s life looks very different. She is sober, medically stable, and living with a sense of control over her health that once felt out of reach. Her story reflects what is possible when comprehensive medical care, behavioral health support, and compassionate relationships come together.
Angela’s journey reminds us why accessible, whole-person care matters — and how stability, trust, and time can truly change lives.
