What is Medical Respite Care?
Medical respite care is temporary transitional housing for people being discharged from the hospital who are high risk for re-admission. Through a holistic care model, we seek to assist clients in managing acute and chronic health conditions while achieving their personal goals related to a stable, healthy and sustainable lifestyle.
Creating Places to Heal
- Clients are referred by local hospitals as part of the discharge planning process.
- Our hospital partners fund the program.
Goals for clients in Respite Care include:
- Stable Housing
- Connection to Primary Care, Behavioral Health and Substance Use Providers
- Health education
- Life Skills and Social Supports
- An aftercare plan
How Does Respite Care Support Better Outcomes?
People experiencing homelessness or unstable housing visit Hospital Emergency Departments three times more often than the general population, and experience markedly higher rates of treatable illnesses. Connection to Community Based Organizational Support as part of discharge planning has been shown to reduce length of Hospital Stay and 90-day readmission rates by 50%, according to National Institutes for Health Statistics.

The Medical Respite Program has become certified by The National Institute for Medical Respite Care, a special program of the National Health Care for the Homeless Council! This means that we have completed a rigorous process to demonstrate a high degree of fidelity to nationally recognized principles, standards, and expectations in the field or Medical Respite Care.
