Funded through the Health Resources and Services Administration (an agency of the U.S. Department of Health and Human Services), Sulzbacher/Duval Family Health Center offers comprehensive healthcare services to the community in need. Sulzbacher/Duval Family Health Centers are classified as Federally Qualified Health Centers and as such, are designed to safely and provide full scale medically underserved rural and population. Services are provided on a sliding fee scale based on the patient’s ability to pay, and all insurances are accepted.
